Occupational Therapy Services Referral Process:
  • Self-Referral:
  • Healthcare Professional Referral:
    • We accept referrals from GPs, specialists, and other healthcare providers.
    • Healthcare professionals can download our referral form or contact us directly.
  • NDIS Participants:
    • If you have an NDIS plan, contact your Support Coordinator or Plan Manager to discuss a referral.
    • We can also assist you in understanding how our services align with your NDIS goals.
  • Insurance or WorkCover Referrals:
    • Please have your case manager or insurance provider contact us directly.
    • We’ll work with them to ensure all necessary documentation is in place.
  • Initial Consultation:
    • Once we receive your referral, we’ll contact you to schedule an initial consultation.
    • This allows us to understand your needs and tailor our services accordingly.

  • Personal details (name, contact information, date of birth)
  • Reason for referral
  • Relevant medical history
  • Current medications
  • Any previous assessments or reports

    Our intake process varies depending on your specific circumstances. Key factors include:

    • Your location and licensing state (e.g., QLD or NSW)
    • Whether you’re a learner driver or require a review of fitness to drive
    • If it’s a private assessment or insurance-related

When you contact us, please provide these details so we can guide you through the most appropriate referral process. We’ll explain the specific steps, required documentation, and any preparatory information you might need.

We offer convenient in-home and face-to-face assessments, serving a wide area that spans from Ballina (NSW) in the south to Brisbane and Ipswich (QLD) in the north.Our commitment to accessibility extends beyond these boundaries, as we’re happy to consider service requests outside our primary coverage area upon request. Understanding the diverse needs of our clients, we also provide telehealth services when clinically appropriate, ensuring that distance is never a barrier to quality care. To maintain our high standard of service, travel charges will be applied based on the client’s location, ensuring we can reach those who need our expertise most.
  • Urgent Referrals:
    If your assessment is urgent, please call our office directly on 0452 437 483. We’ll review the possibility of an urgent booking or add you to our cancellation list to accommodate your needs as quickly as possible.
